2025-2026 Player Documents

NOTE:  You will be required to upload a proof-of-age document (birth certificate, passport, etc.) , AND a head-and-shoulders (passport-style) photo of your player as part of the document completion process. 

Please have these electronic files (.pdf, .jpg, .png, .gif) available on your device/computer before starting the document completion process.

2026-2027 Player Commitment Documents
