NOTE:  You will be required to upload a proof-of-age document (birth certificate, passport, etc.) , as part of the document completion process.   

​Also, if your team will participate in the Mid-Michigan Youth Soccer League (MMYSL), you will also need to provide a head-and-shoulders (passport style) photo
